Schedule Appointment

Appointments can be made by e-mail or telephone. Email is generally best. Please indicate if you intend to use insurance, the name of your insurer.  Please note that appointments are exclusively on-line.

My office hours are as follows:

Mondays: 9:00 pm to 6:00 pm
Tuesdays: 9:00 am to 6:00 pm
Wednesdays: 9:00 am to 6:00 pm
Thursdays: 9:00 am to 6:00 pm
Fridays: 9:00 am to 1:00 pm
